Ceredigion’s Palliative Care Team are based at Ty Geraint, Bronglais Hospital, Aberystwyth. The team includes a specialist doctor, community nurses, rehabilitation therapists and pharmacists. They support patients and families in managing their symptoms and the side effects of treatments, psychological distress or complex end of life issues. They also provide specialist advice to other healthcare professionals. The Palliative Care Team work closely with GPs and community nursing teams across Ceredigion to deliver round-the-clock care. Regular outpatient clinics are held but the majority of care is provided in the home.
Following a referral, the Palliative Care Team assess and support any unresolved symptoms including pain management and psychological distress. They work hand in hand with other health care providers including Severn Hospice and voluntary organisations such as HAHAV.
The Palliative Care Team will advise and support the process of advanced care planning; ensuring patients’ wishes and care preferences are heard and appropriately supported. The team also offer bereavement counselling and art therapy. This support is offered one to one or delivered in small, informal groups.
